I am always asked by patients, “What pillow do you recommend?” or “What is the Best Pillow that can help me wake up without a stiff neck?”

Sleep is by far one of the most important things for health and well being!  However if the spine is not supported properly during the large amounts of time spent sleeping in a bed, neck and back pain can result.

I find that most of my patients are not supporting their spines during sleep.  They are either propping their heads up with too many pillows when sleeping on their backs or are not giving themselves enough support when sleeping on their sides.  The reason for this is that most “regular” pillows just don’t cut it.

It is recommended that you either sleep on your back or on your side.  Stomach sleeping isn’t really recommended due to the neck rotating too much.  Sleeping on your stomach frequently can increase your chances of having neck pain and neck problems.

There are many different neck pillows on the market.  All have claims about how wonderful they are and how you will achieve blissful sleep if you purchase them.  The most important thig to consider when selecting a pillow is if it can support proper spinal alignment when sleeping on your side and on your back.  There is only one pillow that I have found that is able to do both and is comfortable.

My pillow of choice is the Therapeutica Pillow.  I have been personally sleeping on this brand of pillow since 1998.  The pillow doesn’t look like your average cervical pillow, because it is designed to accommodate both side sleeping and back sleeping.  You can see in the picture that is has two larger blocks on either end with an area in the middle that is cut out so the neck can be properly supported when lying on the back.  The ends are larger to support the spine when in a side lying position.  When sleeping on your side it is important to have a pillow that is as big as the distance from your shoulder to your neck.  We measure this distance to match you up with the correct size.

A more natural approach for the Treatment of Chronic Low Back Pain is becoming increasing more popular.  New Guidelines from the University of Pittsburgh Medical Center outline a plan for conservative treatment plan before back surgery is considered.  Treatments such as Chiropractic Care and Physical therapy must be administered for at least three months before surgery would be considered.  In the past, medical doctors and hospitals were not as open to conservative care and would recommend only medication, injections, and or surgery.

Chiropractic Before Spine Surgery for Chronic LBP

University of Pittsburgh Medical Center Health Plan mandates conservative care before considering surgery for chronic LBP cases.

The University of Pittsburgh Medical Center (UPMC) Health Plan, a health maintenance organization affiliated with the university’s School of Medicine, has adopted landmark guidelines for the management of chronic low back pain.

As of Jan. 1, 2012, candidates for spine surgery must receive “prior authorization to determine medical necessity,” which includes verification that the patient has “tried and failed a 3-month course of conservative management that included physical therapy, chiropractic therapy, and medication.” Surgery candidates also must be graduates of the plan’s LBP health coaching program. The program features a Web-based decision-making tool designed to help plan members “understand the pros and cons of surgery and high-tech radiology.” It is the first reported implementation of such a policy by a health care plan.

Putting a Clamp on the Soaring Rates of Spine Surgery

According to the December 2011 issue of the UPMC Health Plan Physician Partner Update, which informed participating providers of the new guidelines and the rationale for their implementation, “We feel strongly that this clinical initiative will improve the quality of care for members who are considering low back surgery, and that it will facilitate their involvement in the decision-making process.” The update also noted, “Surgical procedures for low back surgery performed without prior authorization will not be reimbursed at either the specialist or the hospital level.”

A Conservative Strategy for Managing Chronic LBP

PCP discussion related to self-care consisting of rest, ice, compression and elevation (RICE)

Screening for psychosocial factors or “yellow flags” and incorporate behavioral interventions as appropriate with other treatment interventions

Education on self-management techniques – functional ability assessment and education on return to work / usual activity and function

Enrollment and graduation from UPMC Health Plan Health Coach’s Low Back Pain Program (mandatory) which may also include participation in other programs such as weight loss, physical activity, tobacco cessation, depression and/or stress

Early referral to chiropractor or physical therapist, but before advanced imaging, for manipulation/mobilization; stabilization exercises; directional preference strategies – member and/or provider movements that abolish or cause centralization of pain (McKenzie self-treatment repeated movements that centralize pain)

Detailed documentation of extent and response to conservative treatment including chiropractor/physical therapy documentation

Commenting on the UPMC Health Plan guidelines, Gerard Clum, DC, former president of the World Federation of Chiropractic and Life Chiropractic College West, and current executive committee member of the Foundation for Chiropractic Progress, stated:

“The UPMC should be congratulated for its leadership is establishing policies to assure that the least invasive and most likely to be successful care strategies, including chiropractic care, are applied for a meaningful period of time before surgical considerations are made. This decision is both an important recognition of the value of chiropractic care in the acute low back pain environment as well as a recognition of the clinical and economic downsides to spinal surgery in this situation.”

Headquartered in Pittsburgh, the UPMC Health Plan covers insureds in 15 counties in western Pennsylvania. The plan integrates 20 hospitals, 400 doctors’ offices and outpatient sites.

Chiropractic Services: What the UPMC Health Plan Covers

“It is the policy of UPMC Health Plan to recognize chiropractic services and adjunctive procedures as appropriate and consistent with good medical practice and will provide coverage when the services are medically necessary and covered by the member’s benefit plan for the specific indications detailed in this policy. Coverage is limited to medically necessary services provided by a licensed doctor of chiropractic, within the scope of his/her license.”

“Covered chiropractic services include evaluation and management, manipulation, spinal X-rays, therapeutic exercise, and adjunctive procedures that are appropriate and medically necessary for neuromusculoskeletal conditions. … Indications for Chiropractic Services: Indicated for primary, neuro-musculoskeletal symptoms involving the spine, para-spinal soft tissues, and extremities. Indications for Manipulation: Manipulation is appropriate to restore function that has been reduced or lost by illness or injury. Indications for Adjunctive Procedures: Adjunctive procedures are appropriate to restore function and prevent disability following injury. Indications for Therapeutic Exercise: Indicated for improvement or to restore functional status by building strength, endurance and flexibility of the affected region.”

Diet – Nutrition-Food Sensitivity:  This is a pretty big subject area to cover, but I do want to mention it because it is a very important and even some simple dietary changes can make a huge impact on headache relief.  I will just bring up a few key points to consider.  It is important to identify if the patient is eating foods on a regular basis that they may be causing irritation.  Identifying those offender and removing them for a treatment period can be very effective.  In addition to removing foods that cause sensitivities, it is also important to remove harmful foods like sugars, soda, refined foods, processed foods, tobacco, and chemicals etc. that are being consumed.  Headaches can result when the body becomes toxic or poisoned from these substances.  The next thing to address is eating a balanced diet with lots of vegetables and adequate protein and making sure basic supplements like a high quality multivitamin and fish oil are part of that regimen.  DON’T underestimate the power of the food that you put in your body.

Derrick Rose had never before sat for five straight games in his NBA career, so the deafening roar that drowned out his name after public-address announcer Tommy Edwards began “From Chicago …” didn’t surprise.

But when close to the same roar accompanied Rose merely dribbling on the Bulls’ first possession Monday, the impact of his absence became more fully felt.

His teammates didn’t cheer, at least publicly. They merely benefited from Rose’s aggressive presence that featured 23 points, six assists, five rebounds and four turnovers in the Bulls’ 90-79 matinee victory over Atlanta.

“It was awesome to watch,” Carlos Boozer said.

Rose, who had been sidelined by back spasms, logged close to 35 minutes and, in an atypical move, didn’t appear in the postgame locker room to address reporters. He spoke to ESPN’s Doris Burke on the court afterward.

“The whole week I was seeing a great chiropractor,” Rose told Burke. “Teammates and fans were waiting for me to come back. I was feeling good and just trying to play the way I normally play — aggressive.

“My teammates followed me. Coach did a great job with the game plan. (My back) feels good. I have to put a little ice on there. I should be all right.”

Boozer added 16 points for the Bulls, who have won 13 straight regular-season games following a loss. Joakim Noah rebounded from his rough outing on Saturday to grab 16 rebounds, helping the Bulls to a 51-41 advantage on the boards and 18-4 edge in second-chance points.

But Rose dominated attention. He survived a hard tumble after drawing a foul on Josh Smith for a three-point play midway through the third quarter.

“You don’t want him to change who he is,” coach Tom Thibodeau said. “You’re always concerned about health. But a guy could get hurt in practice or shootaround. That’s part of the game.

“He’s done a good job with his rehab. Our trainers have done a great job with him. As long as he’s pain-free and feels 100 percent, we felt comfortable. He met all the criteria that we were looking for him to meet before he played again. So we’re good.”

Actually, not everything was. For the second straight game, the Bulls started slow, forcing Thibodeau to burn a 20-second timeout early in which he blistered the starters. They responded with an 11-0 run as Rose scored nine first-quarter points.

But an eventual 21-point lead with just over 4 minutes to play in the third quarter got quickly whittled to nine by that quarter’s close. Former Bull Jannero Pargo heated up with three 3-pointers.

Then the Hawks, who played without Marvin Williams and with an ineffective Joe Johnson trying to battle through knee soreness, pulled within five in the fourth. Rose almost lost the ball before shoveling it to Luol Deng, whose 3-pointer with 2:59 left marked his first points since the first quarter and provided breathing room.

“Derrick’s explosion was there,” Thibodeau said. “His drives were there. Conditioning-wise, he’s not where he normally is. But that’s to be expected. Overall, I’m very pleased. He said he felt real good out there.”

Neck Pain: Chiropractors, Exercise Better Than Medication, Study Says

When it comes to neck pain the best medicine is no medicine at all according to a new study published in the Annals of Internal Medicine.

The study, funded by the National Institutes of Health, tracked 272 patients with recent-onset neck pain who were treated using three different methods:
A Chiropractor
After 12 weeks the patients who used a chiropractor or exercised were more than twice as likely to be pain free compared to those who relied on medicine.

The patients treated by a chiropractor experienced the highest rate of success with 32 percent saying they were pain free, compared to 30 percent of those who exercised. Only 13 percent of patients treated with medication said they no longer experienced pain.

“Doesn’t surprise me a bit,” Dr. Lee Green, professor of family medicine at the University of Michigan told ABC News. “Neck pain is a mechanical problem, and it makes sense that mechanical treatment works better than a chemical one.”

Dr. John Messmer who specializes in family medicine at Penn State College of Medicine agrees.

“I always prescribe exercises and/or physical therapy for neck pain,” he wrote. “I also tell patients that the exercises are the treatment and the drugs are for the symptoms.”

The exercises prescribed to patients in the study were simple and designed to be performed at home with the help of instructional photos.
